The programme focuses on the following conditions:

Cardiovascular Disease including:
Heart Failure
Ischaemic Heart Disease
Cerebrovascular Disease (Stroke / Transient Ischemic Attack (TIA))
Atrial Fibrillation
As part of this structured programme we provide two scheduled reviews to patients in a 12 month period. These reviews involve patient education, preventative care, medications review, physical examination, investigations including blood tests and formulation of an individual care plan.
